My girlfriend is a smoker (apparently trying to quit), but how can I gently ask her to chew some gum, or a breath mint, or brush her teeth, or SOMETHING before we kiss?

Marry before submission. She will not be eligible for derivative status otherwise and it would take several years for a visa to become available for her.
